There is a new Graduate Manual of Style (2003-2004) posted on the UTA website. 

1.  Go to http://grad.uta.edu/

2.  Scroll down to "Current UTA Graduate Students"

3.  Click on "New Virtual Graduate School Advisor"

This site will lead you to the new Manual of Style, step-by-step sample pages for thesis, and all forms needed for completion of Master's Degree/Thesis.  Plus, the new formatting template for writing.  You'll find all the forms you need, such as your tentative and final programs of work, etc, in electronic format.

However, do not attempt to go through the previous link for the Manual of Style at www.uta.edu/etd/ because that link will lead you to the 1997 version.  The Grad School is trying to kill that link.
