The Office of Talent, Culture, and Inclusion is implementing new clearance and release processes, including a form to be completed by resigning employees, students,
retirees, and transfers. Adjunct faculty with UTA-owned assets must also follow this process.
Beginning March 1, please direct employees resigning
within two weeks to the
My Apps page to complete
the Employee Clearance & Release Request form (icon shown below). Back-dated forms for employees who left the university prior to March 1 do not need to be submitted.

How the form process works:

Resigning employee virtually submits the form to their manager

The manager virtually accepts the form

The employee receives notification of the manager’s acceptance
-
The manager receives clearance checklist items to be satisfied before the employee’s last day
An employee being involuntarily terminated must contact their department’s
Human
Resources Business Partner (HRBP), who will complete the form.
